Stylianou<br \/>\nJune 5, 2018<br \/>\n<\/em><br \/>\nWhen there is domestic violence in the home, children may witness or experience the violence. This can make them feel unsafe and more vulnerable to developing mental health issues such as post-traumatic stress disorder. To help these children, Safe Horizon and our partners have created a groundbreaking pilot program called the Child Trauma Response Team (CTRT). Our Research and Evaluation team has analyzed the effectiveness of the pilot program, and with support from Rutgers University, is sharing our findings with this blog post.<\/p>\n<h1>What is CTRT?<\/h1>\n<p>CTRT is a collaborative model designed to reduce the negative impact of children&#8217;s exposure to domestic violence. From March 2016 to April 2017, a pilot program was implemented in the East Harlem section of Manhattan for families in the 23rd precinct. The pilot program was a partnership with:<\/p>\n<ul>\n<li>New York City Police Department (NYPD)<\/li>\n<li>New York County District Attorney&#8217;s Office (DANY)<\/li>\n<li>Safe Horizon, Inc.<\/li>\n<\/ul>\n<h1>What Does the CTRT Program Do<strong>?<\/strong><\/h1>\n<ul>\n<li>Identify children who have witnessed serious incidents of domestic violence and provide an immediate, coordinated, trauma-focused and interdisciplinary\u00a0response to children under the age of 18 and their impacted family members.<\/li>\n<li>Provide coordinated home-based outreach by the NYPD and Safe Horizon, case management services, and evidence-based, mental health interventions.<\/li>\n<\/ul>\n<h1>What Was the Goal of Pilot Program?<\/h1>\n<ul>\n<li>Determine the impact of the CTRT on children exposed to severe domestic violence and their impacted family members<\/li>\n<li>Identify key factors that facilitate successful collaborations between law enforcement and domestic violence\/child trauma specialists<\/li>\n<li>Provide a roadmap for expanding and adapting the CTRT program throughout New York City<\/li>\n<\/ul>\n<h1>Who Participated in the Study?<\/h1>\n<p>Eligible families were identified by reviewing the NYPD Domestic Incident Reports as well as receiving referrals for the program from a partner domestic violence organization and the Manhattan District Attorney&#8217;s Office.<\/p>\n<ul>\n<li><strong>105<\/strong> families were eligible for the CTRT intervention and <strong>90<\/strong> families elected to receive services<\/li>\n<li>The majority of caregivers were females between the ages of 24 and 39, identifying as Hispanic\/Latina or Black\/African-American and caring for one or two children primarily between the ages of 0 and 6.<\/li>\n<li><strong>82% <\/strong>of families who elected to receive services had at least one child that screened positive for post-traumatic stress disorder<\/li>\n<\/ul>\n<p>Following a severe incident of domestic violence, the Child Trauma Response Team:<\/p>\n<ul>\n<li>Provides a therapeutic acute response to the family within 72 hours of the traumatic event consisting of either a joint home visit by the DV Police Officer and a Safe Horizon Child Trauma Specialist or a phone call by the SH child trauma specialist<\/li>\n<li>Completes a child trauma screening and\/or assessment<\/li>\n<li>Educates the caregiver about common trauma reactions and how his\/her\u00a0child\u2019s behavior may be linked to the child\u2019s exposure to domestic violence<\/li>\n<li>Provides safety assessment and risk management to the family<\/li>\n<li>Links the child and impacted family members to evidence-based mental health treatment services, which may include interventions focused on the peri-traumatic phase such as Child and Family Traumatic Stress Intervention (CFTSI)<\/li>\n<\/ul>\n<h1>Practice Recommendations<\/h1>\n<p>The following practice recommendations are from Safe Horizon&#8217;s Research and Evaluation team. They are based on the evaluation of the pilot program.<\/p>\n<ul>\n<li>Enhance the identification of eligible families by focusing on increasing police documentation on the Domestic Violence Incident Report when a child is present in the home during an incident<\/li>\n<li>Collaborate to increase the speed of home visits to families experiencing severe domestic violence within 72-hours post-incident<\/li>\n<li>Shift the model to address needs across diverse communities<\/li>\n<li>Explore expanding capacity to offer mental health intervention services to young children<\/li>\n<li>Discuss the sensitivity and specificity of assessment tools used<\/li>\n<li>Develop and implement CTRT model guidelines for case review meetings, home visits, and steering committee meetings<\/li>\n<li>Define and clarify roles of each partner and enhance communication between CTRT team<\/li>\n<\/ul>\n<h1>Policy Recommendations<\/h1>\n<ul>\n<li>Consider expanding the CTRT model to other NYC precincts to increase impact of the program<\/li>\n<li>Consider expanding the CTRT model to focus on all incidents of domestic violence, rather than just severe incidents<\/li>\n<li>Consider expanding the CTRT model to other forms of trauma and violence experienced and witnessed by children<\/li>\n<\/ul>\n<h1>Research Recommendations<\/h1>\n<ul>\n<li>Secure additional funding for a longitudinal program evaluation<\/li>\n<li>Understand the impact of the CTRT model among a diverse array of communities<\/li>\n<li>Explore the potential impact of the CTRT model in responding to all domestic violence incidents and\/or all traumatic incidents children may witness<\/li>\n<li>Examine the impact of the CTRT on families\u2019 safety, level of post-traumatic stress disorder, access to community services, ability to utilize emotional coping skills, trust in the criminal justice system, and engagement in the criminal justice system<\/li>\n<li>Understand the impact of CTRT from the perspective of families utilizing the program&#8217;s services<\/li>\n<\/ul>\n<p>We are committed to consistently evaluating the impact of our programs and are proud to report on the pilot\u2019s success. This insightful and actionable analysis brings us one step closer to our vision of a society free from violence. CTRT provides a swift, coordinated, and trauma-focused response to children under the age of eighteen and their impacted family members who are exposed to severe domestic violence. CTRT pioneers a model informed by the Child Development-Community Policing Program (CD-CP); a national model of law enforcement and mental health collaboration.\r\n\r\nCTRT resulted from a partnership between The New York City Police Department (NYPD), the New York County District Attorney's Office (DANY) and Safe Horizon. In 2016, Safe Horizon launched the CTRT pilot program in NYPD's 23rd Precinct in East Harlem and most recently in the 34th Precinct in Washington Heights. In Fall 2017, CTRT will expand into additional precincts in Manhattan, the Bronx, and Queens.\r\n<h1>What Does CTRT Hope to Accomplish?<\/h1>\r\nCTRT aims to address immediate safety needs of children and families impacted by domestic violence and reduce the risk of children developing post-traumatic stress disorder (PTSD). The program provides domestic violence victims with linkages to mental health and community support services and enhances police officers' understanding of children in domestic violence situations.\r\n<h1>How Does CTRT Help Domestic Violence Victims and Child Witnesses?<\/h1>\r\nCTRT is designed to provide children and families with a swift, collaborative and effective response by both law enforcement and mental health professionals in the aftermath of a domestic violence incident. This is accomplished by placing a child trauma responder within a police precinct to follow-up with and offer services to a family when a child has witnessed an incident of severe domestic violence in the home. Within 72-hours of a domestic violence incident being responded to by an NYPD officer, the child trauma responder reaches out to the family to offer services, conduct a trauma screening and engage the family in seeking trauma treatment.\r\n\r\nWhile meeting with a family or victim, the child trauma responder assesses current safety risks. They provide information on resources and services available and screens the child or children for trauma symptoms. The goal of this trauma screening and assessment is to further explore how the domestic violence has impacted the child and assess whether they are at risk for developing PTSD. If the child is at risk for developing PTSD, then the team member will explore the possibility of short-term mental health treatment.\r\n<h1>How CTRT Helps Children and Families Heal<\/h1>\r\nThe CTRT pilot program has already made a difference, especially to Kyra and her son Simon. When Kyra's partner William assaulted her, she filed a police report. After speaking with the Patrol Officer, The CTRT Child Trauma Responder reached out to her. During the initial conversation, Kyra shared that William had abused her throughout their relationship and that Kyra's seven-year-old son, Simon, had witnessed the violence. The case manager conducted a trauma screening and assessment with Simon, who was having a difficult time in school and was aggressive with his classmates and siblings. Kyra was relieved to hear that help was available for both her and Simon. Through the CTRT program, Kyra and Simon participated together in a brief, evidence-based mental health intervention, The Child and Family Traumatic Stress Intervention. Through this intervention, Kyra became more aware of Simon's traumatic reactions and they both learned skills in how to manage these reactions.\u00a0 By the end of this treatment, Simon felt better and has shown improvement in his behavior, both in school and at home.\r\n\r\nThe collaborative CTRT model shows great promise as a core service for children and their caregivers who are exposed to severe domestic violence. Eighty-two percent of families completing a PTSD screen had at least one child who tested positive for PTSD. Based on the success of this pilot, we are expanding CTRT to precincts in three different boroughs.
