Of these survivors, 68.3% were seen by medical providers in the Emergency Department (Lederer & Wetzel, 2014). This data shows that there is a huge opportunity for healthcare providers to help identify trafficking survivors and connect them to life-saving services.

In recognition of this reality, New York State passed a law in 2017 that requires all medical providers to be trained in how to identify the signs of human trafficking in the patient population and create internal policies and procedures for referring patients to service providers upon patient request. Safe Horizon’s Anti-Trafficking Program has launched a special initiative to assist with these life-saving efforts. This specialized training shows medical providers in New York City how they can help trafficking survivors find a way out.

Training Healthcare Providers to Identify and Support Human Trafficking Survivors

*Stephanie Clemente is the Manager of Training and Outreach at ATP and has worked on this effort alongside ATP’s Senior Director, Anita S. Teekah. “The goals of each training are to help medical professionals define and understand the severity and scope of human trafficking, identify red flags of trafficking in healthcare-specific scenarios, understand trauma and the importance of utilizing a trauma-informed lens, and learn practical skills to increase efficacy in supporting survivors of trafficking.”

During the training, Stephanie guides medical providers in understanding the signs and red flags of human trafficking. For labor trafficking survivors, medical red flags could include musculoskeletal repetitive work injuries resulting in chronic pain, respiratory problems from exposure to dangerous materials or lack of ventilation, or compromised eyesight from working in dimly lit workplaces. For sex trafficking survivors, medical red flags could include any of the above compounded by reproductive issues including recurrent sexually transmitted infections, trauma to the genital area, or frequent pregnancies, miscarriages, or abortions.

Another component of the training is helping medical providers understand the trauma that trafficking survivors experience. We also bring in social worker staff to provide a deeper clinical component to the trainings if the provider requests a social worker in-service trainings or advises that social workers will comprise the majority of the attendees. Stephanie focuses on defining and identifying human trafficking among the patient population, including discussion of red flags, vulnerabilities and how to best support a potentially trafficked patient in a trauma-informed setting. “When speaking to a potential human trafficking survivor, medical providers can prevent further trauma by reducing the number of times they interview the patient. They can also hold screenings in non-traditional settings where the patient/survivor feels safe, and have an advocate present to support the patient/survivor as they share their story.” Stephanie explains how being client-centered and respecting survivors’ choice can empower them to stay safe. “Medical professionals may see trafficking survivor patients many times, so we encourage them to offer survivors unbiased support. The survivor may decide to seek help at another point in time, and it’s important to respect any decision the patient makes.”

How Medical Providers Create Change

Medical providers have the power to make a difference. Stephanie and the medical leadership she trains have noticed the impact of these trainings. “At the beginning of each training, I’ll ask if anyone has been able to identify a human trafficking survivor before and not many would raise their hands. But by the end of the training, I’ll ask the same question, and a larger show of hands will confirm that the signs of human trafficking are clearer. I find that most rewarding.”

Stephanie has also received positive feedback from leaders in the medical field. Jaime Devera, Director of Staff Development Nursing & Patient Care Services at NYC Health +Hospitals/Queens has received overwhelmingly positive feedback after Stephanie’s training. “Aside from being engaging and informative, it was an eye-opener for so many,” he comments.

ATP has trained over 800 medical professionals on how to identify and approach trafficking survivors. Many victims don’t know that what is happening to them is actually human trafficking, and therefore don’t self-identify as victims. Red flags for human trafficking include an employer confiscating a worker’s passport or other legal documents, restricting movement, threatening to harm the worker or his/her family, and withholding pay for work provided. Human trafficking is, at its core, about controlling another human being for commercial use, and perpetrators can take many forms, including family members, business owners, government officials, and romantic partners.

Often, people identify or define human trafficking as sex trafficking, and while sex trafficking is a widespread form of human trafficking, it is just one type of trafficking. In fact, labor trafficking is more pervasive than sex trafficking, and most of our ATP clients are identified as labor trafficked. Globally, over 20 million individuals are confirmed labor trafficked survivors, according to the most recent estimates from the International Labor Organization. Some of the industries most afflicted by human trafficking include domestic work, restaurants, hospitality, construction, agriculture, and fashion. It is likely that most, if not all, of us, have encountered a victim of human trafficking, but didn’t know enough to identify the situation as trafficking.

How Does Safe Horizon and ATP Help Victims?

Safe Horizon established the ATP in 2001 in a concerted effort to provide critical wrap-around services for human trafficking victims in the New York City area. Since its inception, ATP has helped more than 1000 victims from over 80 countries. ATP is the largest direct services provider for trafficking victims on the East Coast and offers comprehensive legal and social work/case management services, supportive counseling, limited financial assistance, family reunification assistance and training and outreach for our community partners. Simultaneously, ATP is a member of multiple national, state and local policy and advocacy coalitions, and advocates for strong survivor-centered legislation at all levels of government. Over the last year and a half, we lobbied for reauthorization of the Trafficking Victims Protection Act (TVPA) and attempted to pass legislation that would vacate criminal convictions for trafficking survivors for crimes that their traffickers forced them to commit. We work with all stakeholders to ensure that our anti-trafficking work is informed and comprehensive. Our key partnerships include those with law enforcement from all agencies, service providers and community organizations.

Most importantly, all of our work is client-centered and trauma-informed. The program’s strength is in our broad scope of services, and our holistic approach with all clients. Our extensive history as direct service advocates has helped us develop a level of expertise and engagement that only a limited number of organizations around the country can offer. We take lessons learned from our clients and survivors to formulate policies, strengthen ATP and advance our survivor leadership group, Voices of Hope. Our clients are the experts in their own lives and have been invaluable members in the movement to combat human trafficking.

What Does the Future Hold for Anti-Human Trafficking Efforts?

The political events over the last two years have greatly impacted advocacy and direct service efforts by anti-trafficking organizations, including ATP. Although human trafficking initiatives have historically enjoyed bipartisan support in Congress, we are seeing an anti-immigrant shift that adversely impacts our clients. Over half of our survivors are foreign nationals and have suffered horrific mistreatment at the hands of others. They should be treated as human trafficking survivors entitled to assistance, and not as immigrants to be deported. Nonetheless, the recent reauthorization of TVPA reconfirms the United States’ commitment to preventing human trafficking, protecting survivors and prosecuting trafficking crimes. Much work remains, however, and the next few years will be critical for appropriate implementation and enforcement of TVPA mandates and policies.

As we move into 2019, ATP will continue to fight for sufficient funding and resources to serve all victims of human trafficking, regardless of nationality, type of trafficking or gender. We will also strengthen existing initiatives to increase awareness of labor trafficking and investigation by law enforcement. Criminal justice reforms for our survivors, including vacatur of their criminal convictions, are also critical to establishing their enduring stability post-trafficking. Our strategic membership in leading thought-coalitions ensures that the voices and concerns of survivors remain paramount in all anti-trafficking advocacy, regardless of the political climate in which we are working.

Frequently Asked Questions About Human Trafficking
In this Human Trafficking FAQ, senior director of our Anti-Trafficking Program answers frequently asked questions about human trafficking.

By Connor Cashel
January 9, 2019

As my first interview out of graduate school came to a close, I asked Ileana Taylor, a social worker at Safe Horizon’s Anti-Trafficking Program (ATP), the question that would resonate throughout my entire time at Safe Horizon. “What does a typical day look like as a social worker at the Anti-Trafficking Program?” I asked this, first, because I was genuinely curious. My prior experience consisted of working only with female sex trafficking survivors and I knew this position, should I receive it, would entail very different responsibilities which I irrationally hoped could be described in one short sentence. Second, I asked because Google told me it was a good interview question, and I desperately wanted the interview to go well, as I had aspired to be part of this program since I began working in the anti-trafficking field two years prior.

Connor and Ileana reminisce about their experience working together at ATP

Five months later, I sat with Ileana on a Friday afternoon laughing as we recalled her response to my difficult to answer question. At ATP, I work with all types of trafficking survivors, connecting them to a variety of resources including medical care, legal services, and job training programs, housing, benefits and more. I often prepare them for or accompany them to legal proceedings. I remember working extensively with one survivor in particular. I helped her navigate and manage the criminal justice proceedings taking place against her traffickers. After preparing, I flew with her across the country to provide trauma-informed support throughout the extremely triggering experience of facing her traffickers in court.

I have learned so much about trauma’s impacts on human trafficking survivors. I have learned to help survivors identify their trauma reactions and use trauma-informed interventions and strategies to help survivors manage these reactions. While trying to continuously uphold a trauma-informed framework I have assisted survivors with a range of case management needs. This made it clear that my desire for a short answer from Ileana was even more laughable.

At ATP I also have the privilege of working with other human trafficking service providers and organizations in spreading awareness of human trafficking. I have worked with and conducted presentations to law enforcement officials, crime victim advocates stationed in precincts across the five boroughs, and participated in a briefing with United Nations Special Rapporteur on Trafficking to comment on the needs of trafficking survivors I have noticed as a provider in the field.

My experience at ATP has so far been rich, challenging, and transformative. I am extremely grateful to have colleagues and supervisors who have taken the time to teach me and fully support me in the emotional, and practical difficulties of this position. And I value each survivor who has trusted me to help them through their painful trafficking experiences. I feel privileged to work in an agency, and moreover a program, that has taken the time to invest in my learning and fully support me in navigating the elusive ‘typical day’ which I was so curious about in my interview.
